What You Need to Know
Hackensack Meridian Health (HMH) has created two data reports providing metrics for both REaL data and Sexual Orientation and Gender Identity (SOGI) data. The data is gathered through the Epic Business Intelligence system and is entered by clinicians across HMH in Epic.
The REaL and SOGI data reports are available by campus, unit and date.
The data is sorted to provide information about clinical care related to race, ethnicity, language preference, sexual orientation, gender identity and other socio-demographic characteristics to address disparities and identity opportunities to enhance patient outcomes for diverse populations.
The data can be isolated to a time period or all inclusive to look at metrics over time. The data is used for planning of team member education, patient education, and community outreach programming.
