The National Committee for Quality Assurance (NCQA) has awarded Hackensack Meridian Health Partners (HMHP) its highest level of case management accreditation status.
HMHP is the only organization in New Jersey and one of 49 in the nation to receive NCQA’s three-year accreditation.
HMHP received the designation for the Braven Health Case Management Program – scoring 99.3 out of 100 points.
