Commercial Rosters To Be Available
What you need to know
Hackensack Meridian Health Partners (HMHP) is making commercial payor patient rosters available to our primary care physicians. The rosters will be available online in the coming weeks and will also be hand delivered to select practices. How to access your roster online
To access your roster online, you will need to enter the username and password that you use to access the Hackensack Meridian Health network (ie: Soarian, Epic, etc.). Instructions and a link to your roster will be emailed to you in the coming weeks. Roster review
Independent primary care physicians with a large number of attributed patients will be contacted by a care coordinator from HMHP. HMHP’s care coordinators are clinical professionals who are here to ensure your attributed patients are properly managed in accordance with our payor agreements. Each roster is based upon the attribution methodology developed by HMHP’s physician leadership and includes actively enrolled beneficiaries with Horizon Blue Cross Blue Shield of New Jersey, Aetna, and AmeriHealth. United Healthcare will be added at a later date. We are supplying you with this roster based on physician feedback and in an effort to increase our clinically integrated network’s transparency. Your monthly care coordination payment is based on attributed lives.
