Current members of Hackensack Meridian Health Partners (HMHP) received a letter around March 14, 2019 regarding Hackensack Meridian Health's new team member medical plan.
Please note, members of HMHP who participate in Horizon Blue Cross Blue Shield of New Jersey's (BCBSNJ) PPO will be preferred providers in Hackensack Meridian Health's team member medical plan.
Current HMHP providers who participate with Horizon BCBSNJ do not need to take any action.
Beginning January 1, 2020, Hackensack Meridian Health's team members will be encouraged to receive care from HMHP participating providers as they will receive coordinated care and incur fewer out-of-pocket costs.
