
Care Transformation Services (CTS) is the term used by Hackensack Meridian Health (HMH) to designate its clinical service lines. These service lines are a way of joining similar services around diseases (e.g., cancer, stroke, heart, etc.) or other care dimensions (e.g., OB/GYN, pediatrics, etc.) in an effort to provide seamless, value-based, patient-centered care.

With a Traditional Delivery System there is greater variability of care and performance because there is a siloed and inconsistent approach.
A CTS Integrated Delivery System can achieve superior quality, greater efficiencies, and better care coordination because there is a consistent approach across the network, which results in true “systemness.”
Who makes up the CTS Leadership Team?

The goals of the HMH CTS Leadership Team is to:
- drive standards of care using evidence-based medicine to create metrics, protocols and care pathways to meet both quality and cost goals.
- develop network-wide strategies that will further strengthen the HMH CTSs, resulting in significant program growth and increased access to care.
- support the implementation of network-wide strategies.
- monitor CTS performance metrics such as quality, patient and team member satisfaction, volume, etc.
