- The content of this form is confidential. The completed form can be scanned and then emailed to onelinkteam@hmhn.org or provided to your site’s Risk Manager by interoffice mail or hand delivery. The content of this form will be entered into ONElink after the downtime.
- Please make copies of the paper form as necessary.
