What: The ACP navigator includes a template to document ACP conversations. The template includes
an introductory section "Adult ACP" followed by a choice of three sections, to be completed as
appropriate:
a. Patients with Stable Chronic Illness
b. Patients with Serious Illness (SICG)
c. Patients nearing End of Life
The template for patients with serious illness is based on the Serious Illness Conversation Guide
(SICG). The other templates are tailored to include the most important information for that
patient population.
