HMH’s Social Determinants of Health (SDOH) tool NowPow is now Unite Us. Starting October 31, 2023, Unite Us will beseamlessly integrated into Epic. This update will further empower our care teams to address Social Determinants of Health (SDOH) effectively, from food insecurity to behavioral health needs.To access this tool, team members must complete the “Social Determinants of Health (SDOH) Documentation in Epic & Unite Us Community Resources” training via MySuccess. Training is now available in the LMS as of Monday, October 9, 2023. Unite Us allows efficient SDOH screening in Epic, matching patients to tailored local resources available in 150 languages, delivered via email, print, or text.
