During the Admission Medication Reconciliation process in Epic, a user might attempt to clean up the home medication list by selecting the ‘X’ button from the Review PTA medication section or the ‘Discontinue’ button from the Reconcile Prior to Admission section. By selecting 'X' or 'Discontinue', the system sends a CANCELLATION of home medication to retail pharmacies for e-prescribed orders.
To prevent CANCELLATION of home medications at retail pharmacies, a new Discontinue reason called 'Medication List Clean Up' was created.
Please select 'Medication List Clean Up' when updating or reconciling the medication list to prevent permanently discontinuing prescriptions at retail pharmacies.
