What You Need To Know
Beginning on May 19, 2026, a new IV flush infusion panel will be implemented across all HMH sites to ensure the optimal administration of IV medications diluted as small volume parenteral (SVP - defined as 100 mL or less). This practice is intended to prevent loss of residual medication in IV tubing at the completion of each infusion.
- An OPA will fire when a SVP is ordered, suggesting the Flush Infusion order panel be ordered as well.
- Note: If active orders for flush infusions are already present on the patient’s medication list, the OPA will NOT fire.

- Nurses will flush the SVP using 25 mL of the same carrier fluid as the SVP diluent.
- Example: ampicillin/sulbactam (Unasyn) in 0.9% NaCl infused at 200 mL/hour would be flushed with 25 mL of 0.9% NaCl at 200 mL/hour.
- Pharmacy has been working with Purchasing to ensure that adequate carrier fluids are available. Unit nursing leaders should work with Materials Management to ensure that a sufficient quantity of carrier fluids are accessible for their units (see below for PS numbers).
