What You Need To Know
At Hackensack Meridian Health, patients are encouraged to participate in shared decision making regarding their plan of care, which combines the patient’s known goals, values and preferences with their caregiver’s expertise and knowledge; however, there are times when a patient (or surrogate) refuses the recommended plan of care and decides to the leave the hospital against medical advice. To ensure consistency in managing this across the network, a newly harmonized Leaving Against Medical (AMA) policy and AMA form is in effect as of December 10, 2024.
